Guidelines for Holder of Life Membership ID Card

1. Click Register then scroll down for membership details including requirements
2. Fill up the membership form completely and accurately
3. Upload the requirements (scanned Life Member ID card, scanned 2×2 ID picture and proof of payment – 200 pesos)
4. Tick the Data Privacy Notice
5. Click Submit
6. Wait for the updated Life Member ID to be mailed based in your encoded mailing address.
